Enrollment Instructions
THIS PACKET OF INFORMATION IS ONLY FOR THOSE WHO HAVE ALREADY BEEN ACCEPTED TO VISTA SCHOOL. IF YOU HAVE NOT RECEIVED AN ACCEPTANCE LETTER THEN YOU SHOULD NOT FILL OUT ANY OF THESE FORMS. THIS ENROLLMENT PACKET IS REQUIRED TO BE SUBMITTED BY ALL NEW AND RETURNING STUDENTS BY MARCH 19TH.
This enrollment packet contains the following forms:
- New Student Registration Form
- Release of Permanent School Records Form
- Physical Exam Record
- Immunization/Exemption Guidelines
- Guardianship Status Form
- Disciplinary History Form
- Authorization of Student Medication(optional as needed)
- Directory Release Request Form
- Emergency Contact form
- Parent Standards Agreement Form
- Class Interest Survey Form
Please provide the following:
- Copy of Birth Certificate(one per student)
- Copy of Utah Immunization record/Exemption Form
The above registration forms must be filled out and submitted by all students no later than March 19th. Failure to return these forms by the due date may result in forfeiture of your child’s place at Vista. All forms need to be returned to Vista by one of the following methods. 1) Scanned and emailed, 2) Faxed, or 3) Mailed to one of the following:
